The research study intended to assess the factors affecting annual procurement plan with consideration of its preparation and implementation in public institutions in Tanzania, a case of Mbeya Regional Referral Hospital. The researcher intended to assess the competence of staff in preparation and implementation of annual procurement plan, determine the influence of the financial factors, legal framework and the external factors especially supplier and political influence in of annual procurement plan, at Mbeya Regional Referral Hospital. The research design employed was a case study design which enabled the researcher to obtain and collect data easily in one intended area which was Mbeya Regional Referral hospital. The researcher collected both primary and secondary data. Primary data was through questionnaire and interview method which involved both unstructured and structured questions and secondary data was collected from document, books and journals. The targeted population was 100 employees at MRRH where from them the sample of 80 respondents was used. From the sample the 76 counted to 95% of the sample size return the questionnaires as well as provided their cooperation interview was used by researcher to know more on the annual procurement plan. The data collected from the study was analyzed through descriptive statistics, where frequencies were used to describe the response of respondents. On other side researcher used inferential statistics by employing correlation analysis and regression analysis to test the relationship between the independent and dependent variable and the influence of independent variables to dependent variable, where from the findings the analysis found strong significant relationship between independent and dependent variable ,staff competence contributed to33.8% of effective preparation and implementation APP financial factors contributed to 43.3% on preparation and implementation of APP, legal framework contributed 22.2% of preparation of APP and external factors 14.3% on implementation of APP. On recommendation; the researcher recommends that more studies should be done in drug procurement planning since there is a variation in diseases, fund should be supplied on time also training to staffs and effective cooperation with suppliers for effective annual procurement plan.
