Introduction Satisfaction is a crucial element in utilization of services. Clients adhere to treatment, care and support when they are satisfied. Furthermore, without making clients satisfied with the service and utilize the service effectively HIV/AIDS will remain a problem in the society. Mainly the affected ones are women and children who constitute half of the global people living with HIV/AIDS. UNAIDS reported that among the 34 millions of people living with HIV, 370,000 of them are children that have been infected with HIV were through mother-to child transmission (MTCT)'. Methods A cross sectional survey was done within three health facilities in Kinondoni Municipal council to assess clients satisfaction with regards to waiting time, privacy and other socio-demographic characteristics. Also a study was done to collected health facilities characteristics by getting views of health care workers working in respective health facilities. Results A total of 108 clients receiving PMTCT service in selected three health facilities were interviewed. About 70% of respondents were aged less than 29 years and 61% were married. More than 58% had secondary education as highest level, with 56% being un-employed. Women reported waiting longer had less odds of being satisfied with PMTCT services offered at health facility as compared to women who waited for an hour or less by0.31 [0.09 - 1.08]. Married women had less odds of being satisfied as compared to single women by 0.25 [0.08 - 0.79]. Employed and un-employed clients had both less odds of being satisfied as compared to self-employed with odds as follows The satisfaction was also associated with marital status and occupation with odds ratio as 0.05 [0.01 - 0.45] and 0.08 [0.01 - 0.67] respectively. Overall satisfaction for aspects of privacy during consultation is 20% while overall privacy in respect to privacy during medicine dispensing is 29%. Conclusion Waiting time and demographic factors should be taken into account to increase client’s satisfaction with PMTCT services offered within health facilities in Kinondoni. Privacy during both consultation and medicine dispensing should be improved for better services and even more client’s satisfaction at health facilities within Kinondoni.
